Photographer Agreement

As the Photographer or said representative, I am authorized to share the photographs I am submitting or will in the future submit for inclusion in Photoshare. By signing this form, I hereby grant to The Johns Hopkins University a non-exclusive, worldwide, perpetual, royalty-free license to use such photographs in its Photoshare database, and grant all such other rights, including the right to sublicense, the right to copy, distribute, publicly display or perform, and the right to create derivative works, as may be necessary to meet the requirements of the Photoshare database, and to fully realize and carry out the purposes thereof. On behalf of The Johns Hopkins University and for the benefit of the Johns Hopkins Bloomberg School of Public Health/Center for Communication Programs (CCP), and on behalf of Photoshare and The INFO Project, and I further grant the unconditional right to reproduce, display, share, and disseminate worldwide and in perpetuity, in whole or in part, in any traditional or electronic media format, the photographs described above without requiring CCP to notify me, seek my permission, or owe any form of compensation. Photoshare reserves the right to edit captions in accordance with Photoshare’s protocols and style guide. This agreement will apply to all photographs and other submissions supplied to CCP by Photographer and shall remain in full force and effect until cancelled in writing by either party or superceded by a subsequent agreement. I understand that these images will provide useful information related to people, their living conditions, and their health, and that requests to use the photos will be carefully screened for appropriate and respectful objectives. I confirm that these images were photographed with the knowledge and consent of human photo subjects. By typing my name into the “Signature” field below, I am attaching my electronic signature to and agreeing to the terms above. I understand that if I do not agree to these terms, I should not type my name, discontinue submission of this form, and refrain from sharing photographs for inclusion in the Photoshare database.
